Official Transcripts and Diplomas

Official Transcript Requests

A student can request an official transcript in person in the Office of the Registrar or online. CHSU offers online ordering for secure electronic transcripts as well as for mailed (paper) transcripts through the online transcript service provider, Parchment. Additional fees apply for specialized transcript mailing services (i.e., certified mail, international mail, express mail). 

Students may view their unofficial transcript online by logging on to their student portal in SONIS. Unofficial transcripts do not bear the seal of the University nor the signature of the Registrar and is marked unofficial.

Alumni, current and past students can select to have an official transcript sent electronically, by mail, expediated mail, or picked up in person. The fee for an official transcript to be sent electronically or picked up in person is $10.00. Official transcripts that are mailed or expediated are $10.00 plus additional mailing charges.

Official Transcripts for Licensing and Examination Boards

Official transcripts for Residencies and Licensing Boards are available to order online through Parchment at a charge of $10.00 plus additional mailing charges, if applicable.

Diplomas

CHSU has partnered with Parchment, a digital credentialing service, to print your physical diploma and offer an official digital version of your diploma. Following completion of all degree requirements, students will receive a congratulations email from Parchment asking them to “claim” their digital diploma, which will reside on a secure website but can be easily shared on social media and downloaded as a secure PDF file.

To access your digital diploma, you will need to log on to your existing Parchment account, if you already have an account, or create a new account in Parchment, if you do not have an account. Your digital diploma will always be accessible through your Parchment account.

Students will receive one (1) paper diploma and one (1) official digital version of your diploma free of charge. The size of the paper diploma for the College of Osteopathic Medicine is 11 x 14. The College of Pharmacy and the Master of Science in Biomedical Sciences program diploma size is 8 ½ x 11.

The diploma will list your legal name as it appears on your unofficial transcript located in your SONIS student portal. If your name is incorrect or has changed, you will need to submit a Name Change Request form and supporting documentation prior to graduation to the Office of the Registrar.

It takes approximately two to three weeks from the close of the term for the Registrar’s Office to review that all degree requirements have been satisfied and to officially confer your degree. Once your degree has been officially conferred, your diploma will be ordered and mailed by Parchment.

Diplomas are mailed and emailed directly to students at the most current address on file. To ensure proper delivery of your diploma, Parchment will send email notifications to confirm your mailing address. You will have 72 hours after the receipt of the email to confirm your mailing address. Parchment will also send you an email notification when diplomas have been mailed so you can track your paper diploma. Diplomas are usually received within 4 – 6 weeks.

Please note that diplomas will not be released for any student who has outstanding financial obligations to the university.

Report a Damaged Diploma

If you receive your mailed diploma and it is damaged, please email a photo of the damaged diploma to registrar@chsu.edu so we can order a replacement diploma.

Request a Replacement Diploma

Replacement diplomas may be issued in case of the loss or destruction of the original; or when a name change has occurred. CHSU has authorized Parchment to provide replacement diploma ordering via the web for a fee listed on the website. Information on ordering a replacement diploma is located on the CHSU website. The signatures appearing on the diploma will be those of current University officials and will include a reissue line at the bottom of the document. Replacement diplomas are usually received with 4-6 weeks after placing the order. 

Alumni may place a diploma reorder through their Parchment account.
